Which insurance do you accept?
We accept Aetna Medicare Replacement (No HMO), Blue Cross/Blue Shield PPO, Cigna (no Local Plus or Cigna Medicare replacement), Humana Medicare Replacement (No HMO), PHCS Multi-plan, United Healthcare, US Family Health Plan and traditional Medicare. Please contact your member services prior to your appointment to confirm we are in network with your particular plan.

Where should I park when I come to the office?
You can park in the parking lot for free or in the parking garage attached to our building for a small fee.
Do you validate parking?
No, we do not validate parking.
Where can I get my lab work done? My insurance requires a specific lab.
Please contact your insurance plan to confirm which lab is in network prior to your appointment. You can then notify the nurse so they may complete the lab order for the lab you wish to use. After you receive your lab order, you may go to either of these websites to find a location convenient for you.
How do I schedule an Office Visit or Telemedicine?
Please call 281-469-8414 during our regular business hours of M-F 9 a.m.-12 p.m., 2 p.m.-5 p.m. Our phones are busiest from 9-9:30 a.m. and 2-2:30 p.m., so keep trying if you are unable to get through.
How do I get my medicines refilled?
Please call your pharmacy to request a refill on your medication. Refills are done during regular business hours. We will not refill medications after hours. You must be seen in our office at least every 12 months to have any medication refilled. Office visits are required more often for management of chronic diseases such as diabetes, high blood pressure, high cholesterol, depression, etc. If you are due for an office visit or lab, we may request that this be scheduled prior to approving the refill request.
What is a nurse practitioner?
A nurse practitioner (NP) is a master's prepared advanced practice nurse who is able to examine, diagnose and treat patients. NPs are able to write prescriptions and order tests, and they are supervised by the physicians. You may schedule an appointment to see the physician or the nurse practitioner.
What is MedicAlert and how do I sign up?
MedicAlert is a nationally recognized 501(c)3 non-profit foundation providing members with the medically recognized MedicAlert bracelets or other jewelry choices. If you suffer from a serious medical condition or medication allergies, emergency first responders can call the MedicAlert 24-hour phone number for additional critical information you have provided to them when you sign up for membership. How do I get a handicap parking permit?
Patients that qualify for handicap parking based on their health condition can obtain a handicap parking application form from our office. This form is taken to the court house where you will receive a placard for your rear view mirror. Your provider can help you determine if you qualify for handicap parking.
