We accept Blue Cross/Blue Shield PPO, Cigna PPO and HMO of Open Choice and Open Access (no Local Plus or Cigna Medicare replacement), PHCS Multi-plan, United Healthcare (Choice, Choice Plus, PPO, Select, Navigate and Core) and Medicare. Which hospital do you prefer to use?

Where should I park when I come to the office?
You can park in the parking lot for free or in the parking garage attached to our building for a small fee. The current fee for 0-3 hours as of February, 2011 is $3.50.
Do you validate parking?
No, we do not validate parking.
Where can I get my lab work done? My insurance requires a specific lab. If you know what lab your insurance requires, please notify the nurse at the time of your order. We will do the lab order for the lab you wish to use. After you receive your lab order, you may go to either of these websites to find a location convenient for you.
How do I schedule an Office Visit?
Please call 281-469-8414 during our regular business hours of M-F 9 a.m.-12 p.m., 2 p.m.-5 p.m. Our phones are busiest from 9-9:30 a.m. and 2-2:30 p.m., so keep trying if you are unable to get through.
How do I get my medicines refilled?
Please call your pharmacy to request a refill on your medication. Refills are done during regular business hours. We will not refill medications after hours. You must be seen in our office at least every 12 months to have any medication refilled. Office visits are required more often for management of chronic diseases such as diabetes, high blood pressure, high cholesterol, depression, etc. If you are due for an office visit or lab, we may deny your refill request until those things are done.
What is a nurse practitoner?
A nurse practitioner (NP) is a master's prepared advanced practice nurse who is able to examine, diagnose and treat patients. NPs are able to write prescriptions and order tests, and they are supervised by the physicians. You may schedule an appointment to see the physician or the nurse practitioner.
What is MedicAlert and how do I sign up?
MedicAlert is a nationally recognized 501(c)3 non-profit foundation providing members with the medically recognized MedicAlert bracelets or other jewelry choices. If you suffer from a serious medical condition or medication allergies, emergency first responders can call the MedicAlert 24-hour phone number for additional critical information you have provided to them when you sign up for membership. How do I get a handicap parking permit?
Patients that qualify for handicap parking based on their health condition can obtain a handicap parking application form from their doctor. This form is taken to the court house where you will receive a placard for your rear view mirror. Ask your doctor if you qualify for handicap parking.
